You can do steps 1 & 2 before coming to the outreach clinic or come to the clinic where our Certified Cannabis Consultants will be there to help you with every step of the process.
1. UtahID

2. Utah EVS
Create an account if you don’t have one. https://medicalcannabis.utah.gov/patients/#apply

3. Visit an Outreach Clinic
- Answering questions
- Assistance with EVS application
- Clinic Intake form
- Scheduling a Pharmacist appointment
3. Meet with your Recommending Medical Provider (RMP)
Your licensed medical provider will meet with you during your clinic visit to:
- Review your medical history
- Talk through qualifying conditions
- Determine if medical cannabis is right for you
4. Pay the State Fee
There’s a $8 fee to complete your EVS application. Our team can help you process it during your clinic visit.
5. Receive Your Medical Cannabis Card
Once submitted most new cards can be emailed or printed for you before the end of your visit. Renewal cards are available the next day after expiration typically.
